A Review Feedback Form is a program form and is customizable at the program cycle level.  This article covers what Review Feedback Forms are and where they are created and edited in Next.

Review Feedback Forms

Each award cycle within a program goes through at least one review process where the information that the applicant provides on the Intake form, is evaluated by a reviewer or set of reviewers.  When a program’s award cycle needs to have the applicant’s information assessed in specific ways outside of simple scoring or voting, a custom feedback form is created and used.

The Review Feedback form is only used by reviewers and not seen by applicants.

After reviewers provide their feedback on the custom Review Feedback form, program administrators can view that feedback side-by-side with the applicant’s Intake form to help determine award recipients.

If a program is using a custom Review Feedback form, a best practice is to create that form first, before the review round is set up.

Follow these steps to set up a custom Review Feedback form:

1. Logged in as a Program Owner, select the Open button to the desired program to set up the Review Feedback form.

2. Select the Program Settings link from the left panel menu.

3. Select All Award Cycles in the left panel menu.

4. Select the Open button to the appropriate cycle.

5. Select Review from the left panel menu to reveal the sublinks within this section.  

6. Select Feedback Forms from the left panel menu.

7. On the Feedback Forms screen:

a. Follow the instructions in the Add a Form article to create a new Review Feedback form.

b. Follow the instructions in the Build a Form article to edit a new or existing form.

8. Build or edit the Review Feedback form as needed.  Creating or editing an Intake form with the desired form fields and questions by adding form fields, configuring form fields, reordering fields, and deleting fields is the same process as with other forms.

9. The Review Feedback form will automatically save as changes are made to it.
