Overview
A Request form is a program form and is customizable at the program cycle level. This article covers what Request Forms are and where they are created and edited in Next.
Request Forms
Request forms are the forms that third parties complete in support of a program applicant.
The applicant does not see the Request form, but the applicant does trigger the disbursement of the Request form; the Request form is sent to the designated third parties when an applicant completes a request section on an Intake form.
To understand how the request section is set up for applicants, see the article titled Configure a Request Section.
To create a new Request Form for third parties to complete:
- Logged in as an Org Owner, Program Owner/Manager, select View Program or the three dots Award Cycles to the desired program to set up the Request form.
- Select the Cycle Settings gear icon from the left panel menu.
- Select Intake in the top navigation and then Request Forms on the left navigation.
4. On the Request Forms screen:
a. Follow the instructions in the Add a Form article to create a new Request Form.
b. Follow the instructions in the Build a Form article to edit a new or existing form.
5. Build or edit the Request form as needed. Creating or editing a Request form with the desired form fields and questions by adding form fields, configuring form fields, reordering fields, and deleting fields is the same process as with other forms.
6. The Request form will automatically save as changes are made to it.
Adding a Request Form to an Existing Request Section
- Once the Request form has been set up, open the Intake form to which the Request form needs to be added.
- Select the Request Section field and the dotted outline will appear around the field to configure it.
- Select the gear icon in the upper right of the Request Section field.
- When the configuration panel opens, select the Request Form from the dropdown list.
- Finish configuring the section:
a. Choose which applicant fields are required. An email address is always required.
b. Set the minimum number of completed requests the applicant needs before they can submit. For example, with a minimum of 2, two people must fill out and submit the Request Form for that applicant. If requests are optional, set this to 0.
c. Set the maximum number of requests the applicant can send, up to 10. This is how many people the applicant can send the request to. Setting it higher than your minimum gives applicants backups in case someone doesn't respond. For example, with a minimum of 2 and a maximum of 4, the applicant can ask four people and only needs two to come back.
d. Select Done at the bottom of the configuration panel.
- After the configuration panel closes, select the Save button in the Request Section field to save it.
The Request form has now been added to the Request Section of the Intake form.
