Overview

This guide shows Program Admins how to add an Intake Form in Next. 
 


What is an Intake Form?

An Intake Form is the form applicants fill out during an Intake Round — a stage where you collect information from applicants. An award cycle can have a single Intake Round (the main application) or multiple Intake Rounds (for example, a Letter of Intent before the main application). Each Intake Round needs its own Intake Form.

The Intake Form might include application fields, essay questions, and/or budget information — whatever you need to evaluate an applicant and decide whether they receive an award. 

For applicants, the first Intake Form comes after the Eligibility Form (if the cycle has one) or the Profile Form (if it doesn't).

  • You can build the Intake Form as one continuous page, or split it into sections:
    • Sections group questions by topic — like "General Info" or "Financials" — instead of listing everything in one long form. They also make it possible for applicants to save one section and finish the rest later.

⚠️Important Notes

  • Each Intake Round needs its own Intake Form. You can't assign the same form to more than one Intake Round.
     
  • Third-party requests need a Request Section. If an Intake Round collects information from outside parties — like letters of recommendation — the Intake Form must have sections, and one of those sections must be a Request Section. ➡️ See Configure a Request Section.

Step-By-Step Instructions 

  1. Create a New Form: Follow the steps in ➡️Add a Form to create a new Intake Form.
     
  2. Set Up the Form: Follow the steps in  ➡️ Build a Form to set up the form's structure.
     
  3. Rename the Form: Intake Forms are created with a default name — follow the steps in ➡️ Rename a Form if you want to give it a more specific name.
     
  4. Add Fields: Follow the steps in ➡️Add a Form Field and ➡️Customize Form Fields  to add and customize form fields. For an in-depth overview of each form field type, see ➡️Understanding Form Field Types.  ⚠️The Intake Form will automatically save as changes are made to it.
